Initial Consultation

Your initial appointment will consist of an evaluation and a consultation explaining your diagnosis and treatment options. In some more complex situations, the doctor may need to consult with your physician, your general dentist or other colleagues in order to put together the best treatment plan for you. In that case, a second appointment may be needed to explain your diagnosis and treatment options for you. Also, it is rare that any treatment can be completed at that initial appointment, since any special time and equipment needs have not been evaluated.

Please assist us by providing the following information at the time of your consultation:

  • Your referral slip and any radiographs, if applicable
  • A list of medications you are presently taking
  • If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit. It is often helpful to have your spouse or someone else come to the initial consultation appointment to hear what is planned. This reduces the potential for details to be lost in translation when you get home, and gives you a valuable second set of ears and questions to help you fully understand what is being proposed.

A pre-operative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery. With the use of any sedation (oral or general), you will be required to have someone drive you to and from your appointment and remain with you after you go home.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic heart disease, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)

Radiographs

If your dentist has taken radiographs, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional radiographs are necessary, they can be taken at our facility. We have digital radiography available, which may be used in most situations.
