As a courtesy to our patients, we will be happy to assist you in filing insurance claims for your treatment in our office. We may be able to contact your insurance carrier to verify coverage and benefits. We can predetermine benefits if necessary, and discuss flexible financial arrangements that will work for you. We are not a preferred provider on any dental plan, but are members of Washington Dental Service. We will help you maximize your insurance benefits to the best of our ability. Coverage and payment information provided over the telephone by your insurance carrier is not a guarantee of payment and is subject to change by your insurance carrier. Actual payment is determined at the time of claim processing and dependent on eligibility, frequency limitations, deductible, prior work completed and paid, and plan maximums allowed by your insurance carrier. Please remember that you are ultimately responsible for following your insurance coverage limitations. We will do our best to provide you with the most up-to-date and accurate information available to us, but cannot assume responsibility for your actual coverage.
